Directions:
1) Fill in the top contact information section completely.
2) Choose what cool stuff you want!
3) Change the "Add to order?" boxes on the stuff you
want to "Yes" and input size and any personalization information.
4) Double Check your order from
top to bottom - check for "Add to order" boxes being correct and ESPECIALLY for sizes, typos in the personalization
boxes, etc.
5) Hit the submit button at the bottom of the page. You will receive a confirmation message
and then, in about two business days, you'll receive an invoice of your order and a request for payment. Approve
your order and remit payment ASAP. Then, at an upcoming rehearsal, you'll receive your goodies! It's that
easy!
FAQ:
MULTIPLE ITEMS - If you need to
order more than one item (i.e. member jackets for siblings), you need to fill out one order form for EACH. Because of
the personalization options, we can't put mulitple quantities on a single order. Indicate that you are placing more
than one order in the "Additional Comments" box at the bottom. Also, do all of your orders right in a row
so that we can invoice you for all of them at the same time.
PERSONALIZATION OPTIONS - If more
than one personalization option is listed, the extra fee covers any and all of the available options. (i.e. if its $4.00
extra for Name or Name and Section, you'll pay $4.00 whether you just want your name or you want your name and section).
USING STUDENT ACCOUNT MONEY - Once you have paid for your order, you may submit your receipt
to Oregon Band Boosters, INC. Student Accounts Dept. and they will reimburse you (provided you have enough money in your account).
SHIPPING - If you aren't currently involved actively in an OMAAP program, you may request shipping
by placing a comment in the "Additional Comments" box at the bottom of the form.
